Kottukkaali delves into complex themes such as misogyny, superstition, and the strict caste system, all of which are portrayed through the intense drama surrounding the lives of two Indian families. The story revolves around Meena, played by Anna Ben, who is in love with a man from a lower caste. However, her family is opposed to this union and mistaking her emotional turmoil for possession, they take her to an exorcist. The film opens after these dramatic events, showcasing Meena’s silence, which her family attributes to demonic possession, believing that once she is “cured,” nothing will stand in the way of her arranged marriage to Pandi, portrayed by Soori.
